General Information
- MBA Accounting Interest & CPA Requirements: MBA students can pursue an accounting emphasis within the degree's elective portion of requirements. In order to sit for the CPA exam, however, students will need a certain amount of ACCTG courses/content.
Student Forms
Graduate Transfer Credit Authorization (PDF): Students who are attempting to transfer graduate courses from an institution outside of the University of Utah will need to obtain the approval of their Program Director and submit a completed Graduate Transfer Credit Authorization form.
Independent Study (PDF): Students may wish to study a particular area not offered by their department's current course schedule. These courses are considered independent study and require approval from a faculty member that will supervise the student while completing the course. In addition to faculty support, students must gain approval from the department chair and the associate dean of the David Eccles School of Business. Finally, independent study forms should be submitted to the department for processing.
- Leave of Absence Request Form (PDF): The University of Utah requires all graduate students to be continuously enrolled in at least three credits each fall and spring semester. Students who need to take a break from classes for personal, financial, or medical reasons are permitted to petition for a leave of absence from their program for the duration of a fall or spring term. A minimum of one active semester is required to utilize a leave of absence. Completed forms should be submitted to the student's academic advisor for processing.
- MBA Candidacy Form: Candidacy forms should be completed the semester before you plan to graduate. This form is additional to the form required by the Office of the Registrar to officially apply for graduation.
- MBA Course Substitution and Approval Form (PDF): MBA students may petition for permission to take courses outside of the School of Business to serve as electives and/or substitute for core courses.
Online Student Agreement: Newly admitted students should submit the online student agreement.
Request for Letter of Completion (PDF): Students may petition the Office of the Registrar for a letter of degree completion.
